Event Description
|
It was reported that during the one week post-implant check-up, clinic staff found failures with right ventricular (rv) lead testing.An x-ray was performed and showed lead displacement.The rv lead was attempted to be repositioned, however the physician was unable to advance the stylet.Multiple attempts were made and different stylets were also attempted but repositioning was impossible.The lead was then explanted and replaced. no patient complications have been reported as a result of this event.

It was further reported that when the physician tried to advance the stylet through the right ventricular (rv) lead, it did not advance because the end of the electrode tip was not "stretched" and simply advanced approximately halfway.

It was further reported that the rv lead could not stimulate the right ventricle.A high voltage shock was attempted and there was no response from the ventricle.
