Event or Problem Description
It was reported that on (b)(6) 2023 while performing a dsa (digital subtraction angiography) patient suffered stroke, a non occlusive thrombus in the right distal aca territory and mca inferior division was noticed.Patient was administered 2 mg tpa x 2 and 2 mg integrellin injected in the a3 segment, and 2 mg of integrellin injected in the inferior division of mca m4 segment with improvement seen of distal flow.Study device was not implanted due to treatment of the thrombus. It was indicated that the stroke had causal relationship with the study procedure, and has a possible relationship with to dapt, other stryker devices (including the subject long sheath catheter) and an underlying condition or disease. It was noted that the outcome of the stroke was recovered/resolved on (b)(6) 2023.On the same day, post procedure, patient was extubated with concern for new stroke, taken for ct head, at which time she became unresponsive and hypotensive, required intubated, emergent cta abd/pelvis reveling active extravasation from the r cfa with associated hematoma (retroperitoneal hemorrhage).Patient was transferred emergently to nsicu and vascular surgery was cancelled.Patient initially required phenylepinephrine 100 mcg/min, which was decreased and eventually discontinued once she was transfused with 3u prbc as well as platelets, ffp and cryoprecipitate. It was indicated that the retroperitoneal hemorrhage had causal relationship with the study procedure, and has a possible relationship with to dapt, the subject long sheath catheter and an underlying condition or disease. It was noted that the outcome of the retroperitoneal hemorrhage was recovered/resolved on (b)(6) 2023.According to the site the stroke and retroperitoneal hemorrhage are ¿not related to subject long sheath catheter, however the imr (independent medical review) indicates that stroke and retroperitoneal hemorrhage has a ¿possible¿ relationship to the subject long sheath catheter used during the procedure.According to the site the subject long sheath catheter was inside the patient when the patient experienced stroke but was not inside the patients body when the patient had retroperitoneal hemorrhage. It was reported that the patient passed away due to multi-organ failure on (b)(6) 2023.No further information is available.

Additional Manufacturer Narrative
There are controls in the manufacturing process to ensure the product met specifications upon release. The subject device is not available; therefore, functional testing as well as visual testing cannot be performed. The reported event is covered in the device directions for use (dfu). As well, the risk of the reported event is documented in the risk documentation and there are current controls to mitigate the risk of the as reported event. The reported complaint could not be confirmed, and it could not be definitively determined if the device failed to meet specifications because the product was not returned.Based upon medical review, the harm observed in this complaint is anticipated in nature as per the device risk assessment. As a product related root cause does not apply and the issue is due to a known physiological effect of the procedure and/or patient condition noted within the dfu, product labeling and/or risk documentation files, an assignable cause of anticipated procedural complication will be assigned to the as reported "patient stroke', 'patient complications' and 'patient death'.
