Event Description
|
It was reported that almost one month post implant, the right ventricular (rv) lead was oversensing noise and a setscrew problem was suspected.The physician opened the implantable cardioverter defibrillator (icd) pocket and inspected the rv lead.The setscrew was tight, and the rv lead tip appeared to be fully in the device header.The rv lead was disconnected and reconnected carefully to the device.Since there were no obvious issues noted, it was assumed that there was a contact issue.The device and rv lead remain in use.No patient complications have been reported as a result of this event.

It was further reported that one week after the device and lead were revised, high impedance and noise was noted on the rv lead.The same issues resurfaced again.The device and rv lead were removed and a new device and lead were implanted.
