Event or Problem Description

Revision of competitor nail, left hip.It was reported that the screw that comes with the proximal body broke off in the distal stem.Surgeon was not comfortable with the stem construct lacking full locking strength. All pieces were removed and a new stem construct was implanted.Surgery completed successfully with a delay of approximately 10 minutes.Surgeon is very experienced with the restoration modular system, and was using the final torque tightener when the screw broke.Rep asked spd to retain all parts for return.Otherwise, rep confirmed that no further information will be released by the hospital or surgeon.

Reported event: an event regarding crack/fracture involving a restoration modular body was reported. The event was confirmed through mar.Method & results: product evaluation and results: visual inspection: visual inspection was performed as part of the material analysis report (mar). This inspection indicated: images depicted show the cone body / locking bolt and bowed conical distal stem with fracture location of the locking bolt highlighted by arrows. On visual examination, the fracture was observed through the threaded section of the locking bolt.Shows the locking bolt with fracture location highlighted by the arrow, which occurred at the root section of the thread. The fracture was observed approximately 34 mm from the locking bolt head.Explantation damage was observed to both the cone body and distal stem as shown in stereo microscope images, respectively.Shows the fracture surface of the locking bolt.Surface material damage was observed to the threaded section of the locking bolt near the fracture surface.Material analysis: a material analysis has been performed. The report concluded: review of the cone body / locking bolt, catalogue # 6276-1-019, lot code 88716901 confirmed fracture of the locking bolt.Characterisation using stereo microscopy and scanning electron microscopy confirmed fracture of the locking bolt due to overload.Mechanical damage was observed around the sides of the threaded section near the fracture surface. No manufacturing or material related defects were observed on the device surfaces examined.Clinician review: no medical records were received for review with a clinical consultant.Product history review: review of the device history records indicate devices were manufactured and accepted into final stock with no relevant reported discrepancies.Complaint history review: there have been no other similar events for the lot referenced. Conclusions: a material analysis has been performed. No manufacturing or material related defects were observed on the device surfaces examined. No further investigation for this event is possible at this time.If additional information become available to indicate further evaluation is warranted, this record will be reopened.
