Additional Manufacturer Narrative
A video was provided. The video started with the cartridge tip already inserted into the incision. The lens was difficult to visualize. The clarity made it very difficult to see details.Only one corner of the handpiece was visible; unable to identify. The plunger appeared to have been advanced over the trailing optic edge.As the lens unfolded after the delivery the haptics could be observed tucked in the fold. The trailing haptic was broken-distal area with the broken portion adhered to the leading haptic as it unfolded. The trailing optic edge was also broken/torn and fold lines were observed. The lens damage matches the report description. The lens does not appear to be a company lens.Unable to verify from the video.May be due to quality/color of the video.A qualified cartridge was indicated with an unspecified handpiece. The viscoelastic indicated was not qualified. The root cause for the reported damage may be related to a failure to follow the instruction for use (ifu).Based on review of the provided video a plunger override occurred. The trailing haptic was broken-distal area with the broken portion adhered to the leading haptic as it unfolded. The trailing optic edge was also broken/torn and fold lines were observed.A non-qualified viscoelastic was indicated. The company cartridges are qualified for use with compatible company handpieces for the surgical implantation of company qualified foldable iols.Company foldable iols are qualified for use with an company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The use of an unqualified combination may cause damage to the iol and potential complications during the implantation process. The ifu also instructs to completely fill the cartridge with ovd immediately prior to loading and delivery of the lens.Do not attempt to load the lens without adequate ovd in the device. Not adequately filling the device with viscoelastic will result in inadequate coverage of lens and the lens fold path with ovd, which may result in damage.Fold lines were also observed.Fold lines may occur when the lens is advanced too rapidly, when the or/room temperature is too cold, or if inadequate viscoelastic is placed in the device. The manufacturer internal reference number is: (b)(4).
