Event or Problem Description
It was reported that on (b)(6) 2023, a 23mm sjm regent heart valve w/ flex cuff was selected for an implant. The leaflets were moving normally when tested at the time of device preparation.Plastic surgical instruments and physiological saline were used to test leaflet function. The device was successfully implanted in a patient.Post procedure, it was found that the valve leaflet did not open and close properly and was explanted. The leaflet was tested after explant but did not moved normally. On the same day, a new 23mm sjm regent heart valve w/ flex cuff was implanted as a replacement. The patient took warfarin after procedure and latest inr is 2.5.The patient is recovering.

Additional Manufacturer Narrative
The reported event of valve leaflets not opening and closing normally after implant could not be confirmed.Investigation found no anomalies on the valve leaflets, and functional testing at the time of manufacturing and upon return to abbott indicated the valve functioned normally. This test ensures proper leaflet coaptation and hemodynamic performance.Information from the field indicated that there was resistance or sticking while opening/closing the leaflets after the valve was sutured during the procedure. The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications. The cause of the reported incident could not be conclusively determined.
