Event or Problem Description
Medtronic received a report that the device was used safely and successfully as per the ifu during the procedure. The strain relief migration occurred after the procedure was finished and did not involve the patient in any way. While the physician was inspecting the catheter post-op the strain relief was easily removed from its intended location. No patient symptoms or further complications were reported as a result of this event.

Event or Problem Description
Additional information was received that it appeared that the strain relief was not securely fastened to the catheter hub.No unnecessary pushing or pulling occurred during the procedure/use of the apro that would have unusually weakened the strain relief itself.
