An endurant ii stent graft system was implanted in a patient for the endovascular treatment of a 55m abdominal aortic ulcer. It was reported during the index procedure, the physician tried to advance the 28x28x82 through the femoral artery that was exposed via cutdown. The device would not advance even with rotation. The physician removed the delivery system over the wire however, upon removal noticed the tip was missing.Fluoroscopy to the patient's right groin was performed and it showed the tip was lodged in the patient's iliac artery. The iliac artery ruptured when attempting to remove the tip.The rupture was repaired and the tip was removed surgically.A replacement endurant stent was then successfully implanted.Per the physician the cause was due to oversizing. No additional clinical sequelae were reported and the patient is fine.

Product analysis conclusion: the device returned with the external slider in the home position.Deformation was visible along the graft cover.A severe kink was noted on the graft cover at the stent stop. The taper tip was detached from the device and rotational marks were visible on the surface of the nose cone.A section of the spiral member was protruding from the tip.The spiral member was severely deformed and unravelled at the detached site. The graft was slowly deployed, and it was noted that the deformed spiral member had snagged on a stent ring.A kink and separation were observed at the spiral member at the same site the kink was noted on the graft cover at the stent stop. The reported taper tip detachment was confirmed during analysis.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.

Product analysis conclusion: the tip detachment was likely confirmed; however, the cause of the event could not be determined and the iliac artery rupture could not be assessed from the single non-contrast still image provided.Lack of pre-implant ct's did not allow for assessment of the pre-implant anatomy. It is unknown from the film provided at what point the tapered tip detached from the rest of the delivery system.Procedural angiograms showing attempts to advance the delivery system within the artery and also during the removal of the device from the patient was not provided for further analysis of the event. It is possible that the size of the delivery system relative to the access vessel diameter and the diameter of the previously implanted stent may have contributed to the reported events, but this could not be confirmed from the single still image returned.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report.
