Block a2: age at time of event: the patient age is 38 years old.Block e1: initial reporter city: (b)(6).Block h6: imdrf device code a0406 captures the reportable investigation results of guide catheter detached or separated.Block h10: the returned flexima biliary stent was analyzed, and a visual evaluation noted that the guide catheter was returned out of the push catheter. The guide catheter returned stretched, detached and kinked; these conditions could have contributed to the failure to deploy the stent. The push catheter suture hole was torn, and the touhy borst was not returned. The functional test was not performed due to the conditions in which the device returned.Taking all available information into consideration, the investigation concluded that most likely the reported event and observed failures were due to procedure factors encountered during the procedure.User technique applied by the customer during the procedure may have led to the issues with the guide catheter and push catheter. The observed failures found, suggest that the device may have had difficulties when deploying. Therefore, the most probable cause is adverse event related to the procedure.
