Additional Manufacturer Narrative
H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: the physical device was not returned for evaluation. No photos were provided for review. The investigation is confirmed for the reported incorrect use of the device, as the user stated in the reported event details they inflated the balloon above rated burst pressure and are aware of their incorrect use of the device. However the investigation is inconclusive for the reported rupture, difficulty to remove, and deformed balloon as the device was not returned and no objective evidence was provided to confirm these failures. As per the reported event details, the user knowingly over inflated the balloon causing the balloon rupture. Therefore the likely cause of the reported balloon rupture is the incorrect use of the device. It is also possible the reported rupture may have led to the issues with the deformed balloon material and difficulty removing from the sheath. However the definitive root cause could not be determined as the device and no evidence were provided for review.Labeling review: a review of product labeling documentation (e.G., procedural instructions, indications, warnings, precautions, cautions, possible complications, contraindications, nursing guide, and unit label) did not find any product labeling inadequacy.Per the reported information, the user inflated the balloon above the rated burst pressure. This contradicts the instructions for use in the vaccess instruction for use under warnings, step 4: "do not exceed the rbp recommended for this device.Balloon rupture may occur if the rbp rating is exceeded. To prevent over pressurization, use of a pressure monitoring device is recommended." h10: b5, d4 (expiration date: 01/2026), g3, h6 (device, method).H11: h6 (result, conclusion).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : device not returned.
