It was reported that during a da vinci-assisted radical with lymphadenectomy prostatectomy surgical procedure, the customer contacted the intuitive surgical, inc.(isi) technical support engineer (tse) and reported that the illuminator was not working during the procedure and showed a red flashing light. The customer tried reseating the lamp module and power cycling the system. The surgery was converted to a laparoscopic procedure.Isi followed up with the initial reporter and obtained the following additional information: the initial reporter confirmed that the procedure was converted to laparoscopic surgery only because of the illuminator not working issue.

Based on the claim against the product by the customer noting issue with the illuminator, an investigation was completed to determine the cause of this reported event. An intuitive surgical, inc.(isi) field service engineer (fse) was dispatched to the customer site to further investigate the reported event. The fse was able to reproduce the reported problem. The fse checked and found that the illuminator was giving an error 48245 when lamp module was ignited. The lamp was changed but issue remained. The illuminator was ordered and replaced. The system was tested and verified as ready for use.A return material authorization (rma) was issued to the customer requesting to have the intuitive device returned.Isi has not received the product involved with the alleged issue to perform failure analysis. As such, the probable root cause cannot yet be determined based on the information provided.Additional information is being gathered to determine the contribution of the device to the customer reported issue.A follow-up mdr will be submitted if the product is returned (post failure analysis evaluation) or if additional information is received. This complaint is considered a reportable event due to the following conclusion: the customer converted to laparoscopic surgery after the start of the procedure due illuminator not working. While there was no harm or injury to the patient, system unavailability after the start of a surgical procedure could likely cause or contribute to an adverse event if it were to recur as it may lead to an injury due to the patient¿s inability to tolerate a conversion/abortion.
