This report is being submitted as follow-up no.1 to update section h3, and to provide the completed investigation results. One precision device was returned for product evaluation. The returned sample included the dilator and the pouch. The sample was subjected to visual analysis. The dilator was found to be broken 12.2 cm from the hub.The tip piece was measured to be 3.7cm.There appeared to be a hollow metal rod in the dilator shaft.X-ray images were taken, and the metal rod was found to span the full length of the dilator shaft attached to the hub.The rod was from the point of the breakage to a few millimeters to the caulking pin.The complaint can be confirmed for dilator mechanical separation. The exact root cause cannot be determined. The likely root cause is the user mishandled the dilator during insertion through the sheath valve. The device history record (dhr) review determined that the device was in a conforming state when released from terumo control. There is no indication that any manufacturing issues may have led to this event.Currently no action is recommended since this risk evaluation is within the predetermined limits in the failure mode and effects analysis (fmea).
