Additional Manufacturer Narrative
Block h6: imdrf device code a0501 captures the reportable event of distal tip detached.Block h10: investigation results the returned gold probe needle was analyzed. The visual evaluation found that the tip was returned detached from the working length. In addition, the working length bent in different sections. No other problems were noted. The reported event was confirmed.Product analysis identified that the tip was returned detached from the working length (still attached to the device) and working length was bent in different sections. Most likely the manner that the device was manipulated could have resulted in the detachment of the tip. Also, it is possible that procedural factors as lesion characteristics, handling of the device, the technique used by the physician (force applied) could have cause the bents encountered in the working length.Based on all available information and analysis of the returned device, the most probable cause is adverse event related to procedure because the adverse event occurred during the procedure and the device had no influence on the event.According to the information provided, it was reported that the gold probe was used in a pulmonary case (airway/lungs).However, as per the ifu, this device is indicated for the endoscopy procedure. For this reason, this event is cataloged as "cause traced to intentional off-label, unapproved, or contraindicated use" because the problems traced to the intentional use of the device in an unapproved procedure, for an unapproved patient, or for which it is contraindicated, or not listed on the label.
