Additional Manufacturer Narrative

|
Investigation: checking the manufacturing charts of the lot numbers of the components implanted in the previous surgery, we have found the following outcomes: - no pre-existing anomaly was discovered in the (b)(4) items belonging to the lot 1814787 and sterilization (b)(4).- no pre-existing anomaly was discovered in the (b)(4) items belonging to the lot 1815323 and sterilization (b)(4).- no pre-existing anomaly was discovered in the (b)(4) items belonging to the lot 1814146 and sterilization (b)(4).- no pre-existing anomaly was discovered in the (b)(4) items belonging to the lot 18at1ru and sterilization (b)(4).- no pre-existing anomaly was discovered in the (b)(4) items belonging to the lot 1811263 and sterilization (b)(4).According to the information received by the complaint source, the patient had poor bone quality. Moreover, the surgeon decided to go off-label in primary surgery ((b)(6) 2020), using an uncemented application. In fact, in the ifu valid at the time of primary surgery it was stated that the components used in the tema elbow system were intended for cemented applications. This circumstance could have contributed to the necessity of the subsequent surgery, to implant the axle.Hence, considering that: - no pre-existing anomaly was found in the items belonging to the same product codes and lot numbers as those involved in this event.- based on the information received, the surgeon decided to implant the tema elbow components cementless in primary surgery, and this could have contributed to the additional surgery hereby reported. We can conclude that the event is not product related.Pms data: as the surgeon chose to use the implant off-label consciously, no occurrence rate of similar events can be estimated. The manufacturer will continue monitoring the market to promptly detect any further similar issue.Note: this is a final mdr.
