One resolute onyx rx coronary drug eluting stent (des) was implanted to treat a moderately tortuous, mildly calcified lesion with 90% stenosis in the mid right coronary artery (rca). There was no damage noted to the packaging. There were no issues noted when removing the device from the hoop/tray.The device was inspected with no issues noted.Negative prep was not performed. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during delivery. The device was not kinked and re-straightened during use.It was reported that during a diagnostic angiogram it appeared the stent had fractured since implantation thirteen months ago.It was noted that the stent had not detached however, there appeared to be a gap within the 30mm stent. The patient is alive with no injury.

Additional Manufacturer Narrative
Image analysis: the images provided from the account show an angiographic view with contrast injections from different views of the rca where the stent was previously deployed in the proximal rca. There does appear to be a gap at the proximal end of the stent. But based on the stent design, where not every part of the stent wire contains a weld, it appears more likely based on the images and the movement of the artery that the previously adjacent stent wires may have moved apart due to the movement in the artery. This appears to be more stent deformation in vivo than stent fracture. No images showing the original condition of the vessel and the stent profile immediately post deployment were provided, therefore, this cannot be conclusively confirmed.Correction: b.1.Adverse event or product updated.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.

Additional Manufacturer Narrative
Additional information: there were no complications or difficulties noted during the implant procedure. There was one inflation of the stent during implantation at 12 atm's for 15 seconds.Post-dilation was performed at 16 atm's.The stent fracture was identified but no other deformation was noted to the stent.Intervention was not carried out as a result of the fracture event, however, another stent was needed distal to the stent that had fractured. The patient is alive with no further injury.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report.
