Additional information: h6 (component code, type of investigation, investigation findings, investigation conclusions). Results of investigation: the associated devices were returned and evaluated.A visual inspection of the returned oxinium femoral head reveal multiple scratches and signs of wear. The visual of the constrained acetabular liner reveals deep gouges, scratches and discoloration on the device. Also the visual reveal the ring is broken. These devices show signs of wear.A dimensional evaluation reveals the surfaces of the implant are visually damaged due to the attempted implantation.Plastic is easily distorted during implantation activities. These surface finish defects (dings, scratches, scuffs, rubs) are enough to alter the measured reading during evaluation. For this reason, the cause of the complaint cannot be confirmed as the result of a non-conforming part.A review of complaint history for the part number over the past 12 months and for the batch number based on historical data of the device did not reveal similar events for the listed device. The clinical/medical evaluation concluded that the three provided x-ray photos were reviewed and support the reported dislocation but do not indicate a root cause, though the 25-feb-2023 x-ray shows the implant head appearing to be disassociated/dislodged from the acetabular cup.The returned explants reveal a broken ring, but this doesn¿t appear in the x-ray noting the dislocation. With the information provided, the clinical root cause of the reported dislocations and subsequent revision cannot be definitively concluded. However, the reported ¿malfunctioning locking system, the patients¿ history of multiple hip surgeries and/or history of complete absence of the greater trochanter with loss of abductors are all possible contributing factors to her clinical status. The patient impact beyond the revision cannot be determined. It has been communicated; the patient¿s current health status is ¿healthy patient.¿ no further clinical assessment is warranted at this time.A review of the manufacturing records did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident. At this time, we do not have reason to suspect that the product failed to meet any product specifications at the time of manufacture.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.Assessment of historical escalated cases concluded that there are no prior actions related to this device and failure mode.A review of the instructions for use for knee systems revealed in possible adverse effects that dislocation can occur.Factors that could contribute to the reported event include traumatic injury, patient anatomy or abnormal loading of limb.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor future complaints and investigate as necessary. We consider this investigation closed.Corrected data: d9, h3 (device returned to manufacturer), d10 (additional concomitant products added), h6 (health effect - clinical code, medical device problem code).
