Additional information provided in h.3., h.6., and h.10.The product was not returned.A photo was provided which showed the company preloaded device. The plunger lock and lens stop were removed. The plunger was removed from the device and was shown laying to the right of the barrel assembly. No damage observed. The barrel assembly was shown from the side. The lens was visible advanced into the nozzle entry area.Due to the side view, the trailing haptic position could not be visualized. The leading haptic appeared to be extended. This may indicate a plunger override occurred.Unable to determine viscoelastic amount from the photo.A non-qualified viscoelastic was indicated. The product was not returned. The root cause for the reported issue may be related to a failure to follow the ifu (instructions for use).A non-qualified viscoelastic was indicated. The plunger had been removed from the device in the provided photo. The plunger position in relation to the lens during advancement cannot be determined. The ifu instructs: during device preparation and implantation of the company iol with the company preloaded delivery system, an company qualified ophthalmic viscoelastic device (ovd) should be used. The use of an unqualified ovd may cause damage to the lens and potential complications during the device preparation and implantation steps.Viscoelastic amount could not be determined from the photo. The ifu also instructs: fill the device until ovd can be observed flowing to the ¿fill-to¿ line on the nozzle tip. This will require approximately 0.2 ml of ovd.Only use an company ovd qualified for use with the company pre-loaded delivery system that has been allowed to come to the operating room temperature. The manufacturer internal reference number is: (b)(4).
