Event or Problem Description
It was reported that an arteriotomy closure of the left common femoral artery was attempted using a starclose device after a diagnostic procedure with a 6f sheath.Reportedly, the clip deployed, but after a 2 minute groin hold, there was continued bleeding. The physician noted an audible "thug" sound and not the usual distinct/crisp sound expected when deploying starclose. The clip remained in the patient.Manual arterial compression was used to achieve hemostasis. There was no adverse patient sequela and no reported clinically significant delay in the procedure or therapy. No additional information was provided.

Additional Manufacturer Narrative
Analysis was performed on the returned device. The reported failure to achieve hemostasis or the reported noise (the physician noted an audible "thug" sound) could not be confirmed as the exact conditions encountered by the device during the procedure could not be replicated in the test environment.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.Based on the returned device condition, a conclusive cause for the reported difficulties cannot be determined.Factors that contribute failure to achieve hemostasis may include, but not limited to, user pulls back on the device during clip deployment, user rocks/twists the device during device withdraw, clip interacts with the vessel locator wings. It is possible that interaction with the clip during device removal post deployment contributed to the reported "thug" sound. The subsequent treatments appear to be related to procedure circumstance.Based on the information reviewed, there is no indication of a product quality issue with respect to manufacture, design or labeling of the device.
