Type of Reportable Event Serious Injury
Event or Problem Description
Patient underwent a tha (pinnacle corail wax/poly) in 2018 and the implants failed, polyethylene fracture + contact of the ceramic head with the metal of the acetabular implant. The patient complained to the surgeon about discomfort when carrying out his daily activities, as his prosthesis was rigid and making noise. The loosening of the polyethylene liner of the acetabular component was diagnosed.During the revision procedure, breakage of the polyethylene ards (anti-rotational devices) was verified, which promoted the loosening and early deformation of the implant in question. The doctor replaced the damaged components and there was no damage to the patient.Doi: (b)(6) 2018.Dor: (b)(6) 2023.Affected side: unknown.

Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: according to the information received, ¿patient underwent a tha (pinnacle corail wax/poly) in 2018 and the implants failed, polyethylene fracture + contact of the ceramic head with the metal of the acetabular implant. The doctor replaced the damaged components and there was no damage to the patient.Note: the invoice is attached, paying attention to the fact that the invoice was issued directly from j&j to the hospital and not by the distributor, however, as the distributor represents j&j, the doctor requested our assistance.¿ the product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the rim of the pinn mar +4 10d 32idx50od has fractured and some anti-rotation device tabs had sheared off, the broken fragments were returned for evaluation.Deformation was also found on the edge of the liner which indicates that it was loaded by the femoral head and patient's body weight.Additionally, there are machining lines from the manufacturing process yet visible within the liner which would not be as obviously present if the femoral head had been more centrally loaded. With the information provided is not possible to determine a potential cause at this moment, however in support of the evaluation performed, the observed damage of the pinn mar +4 10d 32idx50od may have been caused by exposure to irregularities in the weight loading of the device and moments generated. The mode of failure of the device is multi-factorial and consideration must be given to all other potential influences such as surgical process, patient variables (e.G.Activity level and use), anatomical considerations and patient changes over time.Any conclusions from the investigational input provided have to be placed into context with all other relevant factors.A manufacturing record evaluation was performed for the finished device [121932150 / h84462] number, and no non-conformances / manufacturing irregularities were identified during manufacturing. The overall complaint was confirmed as the observed condition of the pinn mar +4 10d 32idx50od would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation was performed for the finished device [121932150 / h84462] number, and no non-conformances / manufacturing irregularities were identified during manufacturing.Device history review: a manufacturing record evaluation was performed for the finished device [121932150 / h84462] number, and no non-conformances / manufacturing irregularities were identified during manufacturing.H3.
