Event or Problem Description

It was reported that a 61-year-old caucasian female patient underwent a breast augmentation revision surgery with implantation of a 360cc mentor memorygel breast implant prosthesis.Post-operatively, the patient was diagnosed with swollen right-sided lymph nodes. In addition ultrasound imaging was conducted, and it was suspected that there was a ruptured right breast implant.Mammogram imaging was conducted, and the patient was diagnosed with a ruptured right breast implant which was complicated by granuloma formation in the right axilla region.Additionally, she was previously diagnosed with bilateral capsular contracture of undisclosed baker grade ratings. As a result of the contractures, the patient underwent surgical intervention to remove her breast capsules. However, during this procedure, the surgeon explanted the devices and reinserted them.Mentor memorygel breast implants are not intended for reuse/reimplantation, thus the user used the device in error. This report is for the left breast prosthesis.Refer to manufacturing report number 1645337-2023-04661 for the contralateral event.

Additional Manufacturer Narrative

On august 30, 2023, mentor completed an evaluation on the returned device.Mentor conducted a visual inspection of the device.During visual evaluation no apparent damage or visual anomalies were observed on the breast implant.Mentor concluded that the capsular contracture in the patient´s breast was the result of the body´s individual physiological response to the implantation of a foreign object in soft tissue.Capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. The american society of plastic surgeons recommends and encourages member surgeons to always submit breast implants, capsule, and effusion to pathology for examination.Per mentor instructions for use (ifu), these devices are for single use only and should not be re-implanted.Re-use includes a risk of infection (microbial as well as viruses and transmissible agents) as well as immune responses. The sterility of the device can no longer be guaranteed.Manufacturer¿s reference number: (b)(4).
