Event or Problem Description
Information was received from the healthcare provider (hcp) regarding patient receiving baclofen via an implantable pump. The indication use was for intractable spasticity. The healthcare provider (hcp) reported that they refilled their pump on (b)(6) 2023 and changed the drug concentration from 4000 mcg/ml to 2000 mcg/ml.It was noted that she did a bridge bolus, and the patient was discharged. The hcp realized the bridge bolus and simple continuous dose was programmed inaccurately. She programmed the wrong dose which made the bolus run in over 11hrs and 45min.Based on calculations, the bolus should have run in over 110 hrs.The patient will go back to the clinic on (b)(6) 2023.She was looking at the report wrong and noticed the duration was 110 hours and 48 minutes. It was confirmed the only programming that needed changing was the simple continuous dose from 840mcg/day to 420mcg/day.No symptom was reported.
