Event or Problem Description
It was reported that on (b)(6) 2023, 100mm trochanteric femoral nail advanced(tfna) lag screw got stuck in insertion tube during insertion. The interface with t-handle inserter and lag screw toggles and doesn¿t sit properly. The lag screw threads caught the inserter causing the threads to strip and difficulty passing through guide tube.Procedure was completed successfully with a ten(10) min surgical delay. No patient consequences. This report is for one (1) tfna fenestrated screw 100mm - sterile. This is report 1 of 2 for complaint (b)(4).

Additional Manufacturer Narrative
Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report. If the information is unknown, not available or does not apply, the section/field of the form is left blank.H3, h4, h6: part number: 04.038m200sp; lot number: 2452p98; part manufacturing date: 10 october 2022; manufacturing site: elmira; part expiration date: n/a; nonconformance noted: n/a.A review of the device history record revealed no complaint-related anomalies. The device history record shows lot 2452p98 of tfna fenestrated screws was processed through the normal manufacturing and inspection operations with no rework or nonconformances noted. This lot met all dimensional and visual criteria at the time of manufacture with no issues documented during the manufacture that would contribute to this complaint condition.A review of the raw material device history record(s) determined the raw material lot 62p2847 met all specifications with no issues documented that would contribute to this complaint condition. The product was returned to depuy synthes for evaluation. The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample confirm the reported allegation.Signs of deformation were found on the edged and internal surface of the proximal mating connection.Additionally, the threaded tip of the tfna fenestrated screw 100mm - sterile was found stripped with several nicks on the distal portion. The functional test revelated that the device was not able to assemble its mating device properly due to the internal damage of the screw.A dimensional inspection for the tfna fenestrated screw 100mm - sterile was unable to be performed due to post manufacturing damage. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces during the insertion/assemble process. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications. The overall complaint was confirmed as the observed condition of the fna fenestrated screw 100mm - sterile would contribute to the complained device issue. There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
