Event or Problem Description
A facility representative reported that during an intraocular lens (iol) implant procedure, the lens was in fragments/pieces after implantation.The surgeon cut the lens out and safely implanted a new one during initial procedure.There was no patient harm reported.Additional information has been received and stated that, there was no patient harm reported to the patient.
