(b)(4).Date sent: 7/17/2024.D4: udi: as the lot number for the device involved in the event was not provided, the full udi is currently not available.Photo analysis: this is an analysis of a set of images submitted for evaluation.A series of images in pdf format were submitted for evaluation, representing the patient's right upper back (one image included the right upper arm). The photos are associated with (b)(4) and were taken between (b)(6) 2023, and (b)(6) 2023 according to the image tags.In the first photo dated (b)(6) 2023, a tear-shaped skin lesion is visible on the patient's right shoulder blade area. The center of the skin lesion is pale to white, and the surrounding skin is red to dark red.No significant swelling or blistering was observed.Skin lesions look like full-thickness thermal burns (third degree).Additionally, a small burn area was visible on the back of the right upper arm.Due to the poor quality of the image, the burns may have been second degree, but there are no further images after (b)(6) 2023 showing the right arm area.In multiple photos taken between (b)(6) and (b)(6), the central area of the skin wound on the right scapula appears burnt, and the surrounding area begins to heal.A total of 13 photos were taken between (b)(6) and (b)(6) 2023, showing the healing process of the skin wound on the right scapula.By (b)(6) 2023, photo showed evidence of scar tissue formation. In the remaining photos taken between (b)(6) 2023, and (b)(6) 2023, hypertrophic scarring is visible within the original wound area.No conclusion could be reached as to how this issue occurred through photo analysis.A manufacturing record evaluation was performed for the finished device batch number, and no non-conformances were identified.Because the instrument was not returned our evaluation is limited.Additional information received: patient and his mother alleges patient underwent adenoidectomy with bilateral ear tubing which was successful; however, when the patient was in recovery was noted to have suffered two burns, one on his right arm (5cm x 5cm) and the other a ¿baseball size burn¿ on his right scapula (3cm x 1.5cm).The burns have been treated with ointments and steroid tape that will be used for one year.
