The patient was undergoing a coil embolization procedure in the internal iliac artery (iia) using a lantern delivery microcatheter (lantern), ruby coils, and a 5f catheter.During the procedure, the physician successfully implanted six ruby coils into the gluteal arteries using the lantern.Next, while advancing a new ruby coil through the lantern, the physician noticed via angiogram that the ruby coil was protruding from the lantern before reaching the distal end. Therefore, the lantern containing the ruby coil was removed. It was reported that the distal end of the lantern was fractured, but remained connected by an inner wire. The procedure was completed using a new lantern, the previously removed ruby coil, and five pod packing coils (pod pc). There was no report of an adverse effect to the patient.

Evaluation of the returned lantern confirmed a fracture on the catheter. This damage typically occurs if the device is advanced against resistance during use.Evaluation also revealed a kink on the distal shaft. If the device is advanced against resistance, damage such as kinks may occur.Subsequently, if a kinked device is further manipulated, the kink may worsen to a fracture.Penumbra catheters are inspected during in-process inspection and during quality inspection after manufacturing. The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.H3 other text : placeholder.
