On september 01, 2023, mentor received additional information.Mentor became aware that the patient experienced bilateral capsular contracture of an unspecified baker grade. The patient's prosthesis was replaced with a 545cc mentor memorygel boost breast implant.Magnetic resonance imaging identified a trace of peri-implant fluid in the left breast.During the removal surgery, it was reported that 10cc of dark-colored seroma fluid was observed in the left breast. The left-sided capsule was removed and sent to pathology, along with the fluid, for anaplastic large cell lymphoma (alcl) and cd30 positive testing. No further information was provided. On september 12, 2023, mentor received additional information. The patient's date of implantation was updated to (b)(6) 2007.The identity of the impacted product was updated, as clarifying information was received.Size: 500cc brand name: mentor memorygel breast implant lot: 5670973.Catalog: 3545007.Serial number: (b)(6).Expiration date: 3/26/2011.Udi: (b)(4).Manufactured date: 1/16/2006.A manufacturing record evaluation (mre) was performed for the updated lot number, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Manufacturer¿s reference number: (b)(4).

On september 27, 2023, mentor received a device for evaluation. On september 28, 2023, mentor completed a visual inspection of the returned device.Device evaluation summary: visual analysis of the returned sample revealed that the breast implant have a tear on the posterior view within an area of siltex cracking, measuring approximately 2 cm.The evaluation determined that the possible cause of the rupture is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.Mentor concluded that the capsular contracture in the patient´s breast was the result of the body´s individual physiological response to the implantation of a foreign object in soft tissue.Capsular contracture is a known complication associated with these devices and is referenced in our current product insert data sheet. The american society of plastic surgeons recommends and encourages member surgeons to always submit breast implants, capsule, and effusion to pathology for examination.A seroma is a build-up of fluid around the implant.Symptoms from a seroma may include swelling, pain, and bruising.A seroma may occur soon after surgery, or any time later on, which would be referred to as a late seroma. While the body may absorb seromas, some may require surgery for drainage. You should consider the possibility of bia-alcl when a patient presents with late onset, persistent peri-implant seroma. In some cases, patients presented with capsular contracture or masses adjacent to the breast implant. When testing for bia-alcl, collect fresh seroma fluid and representative portions of the capsule, and sent to a laboratory with appropriate expertise for pathology test to rule out alcl, including immunohistochemistry testing for cd30 and alk (anaplastic lymphoma kinase). If your patient is diagnosed with peri-implant bia-alcl, develop an individualized treatment plan in coordination with a multidisciplinary care team. The national comprehensive cancer network (nccn) recommends surgical treatment that includes implant removal and complete capsulectomy ipsilaterally as well as contralaterally, where applicable.Manufacturer¿s reference number: (b)(4).
