H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.Based on a review of available complaint information and an evaluation of the available sample evidence, the following was concluded: the complaint of leakage at the luer adapter was confirmed; however, the root cause was not identified. The product returned for evaluation was one 4fr d/l powerpicc provena catheter.Light usage residues were observed and needleless injection caps were attached to both luer adapters. An attempt to infuse water through the sample using a 12ml syringe revealed both lumens to be patent to infusion and aspiration; however, during pressurization, a leak was observed at the interface between the purple luer adapter and the needleless injection cap.No leaks were observed when pressurizing the purple luer adapter using various non-complainant accessories.Microscopic inspection of the purple luer adapter did not reveal any damage or deformity.Inspection of the leaking injection cap also did not reveal any damage or deformity. The observed leak appeared to caused by incompatibility between the catheter and the injection cap; however, no damage or deformity was discovered.Consequently, this complaint is confirmed as ¿cause unknown¿ at this time.H3 other text : evaluation findings are in section h.11.
