Event or Problem Description

It was reported that the procedure was performed to treat a lesion in the proximal left superior femoral artery (psfa) with moderate calcification and none tortuosity. The 6.0x60mm absolute pro self expanding stent system (sess) was advanced and the stent was implanted. After implantation, it was observed that the proximal and distal portions of the stent were not fully deployed. The 5x40mm armada 35 percutaneous transluminal angioplasty (pta) catheter was advanced for post dilatation. The balloon was inflated once at 12 atmospheres.During removal, resistance was met with the stent struts.Several attempts were needed to remove the device. There were no reported adverse patient sequela and no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
Visual inspection was performed on the returned device. The reported difficulty removing the device from the stent could not be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaint from this lot.The investigation determined the reported difficulty removing the device from the stent appears to be related to operational context. It was reported by the account that the proximal and distal portions of the absolute pro self-expanding stent were not fully deployed. In this case, it is likely that during post dilatation of the absolute pro stent, interaction with the stent struts likely resulted in the reported difficult to remove. There is no indication of a product quality issue with respects to the design, manufacture, or labeling of this device.
