Event or Problem Description
The healthcare professional reported that during a coil embolization procedure for an emergent bleed, after the microcatheter, a marvel® microcatheter (tokai medical products) approached the site of the bleed and the complaint coil, a 3.5mm x 9cm galaxy g3 (gly123509 / 30397648) was delivered. It was reported that the microcatheter kicked back and the coil was unable to be wound as intended. As a result, the coil was withdrawn, but it could not be re-sheathed.Continuous flush was not maintained. The procedure was successfully completed using other coils. There was no negative impact to the patient. On 10-jul-2023, additional information was received. The information indicated that the coil failed to conform to the aneurysm wall; it did not herniate nor protruded into the parent vessel. No neck remodeling was used.It was not necessary to remove the microcatheter from the target when the coil was withdrawn.Based on the additional information received on 10-jul-2023, the event has been deemed usfda reportable under 21 cfr 803 with a classification of ¿malfunction.¿.
