Event or Problem Description
Information was received from healthcare provider (hcp) via a manufacturer representative regarding a generator. It was reported that the tonsil blades on these handpieces were clogged and looked like they were almost melted closed, and were removed from the packaging in this condition. This was discovered during a procedure. Both were disposed of and were not available for return. There was no delay and no impact on patient outcome.

Event or Problem Description
Additional information was received. It was reported that the issue was discovered pre-operatively and there was no patient involvement. The procedure was scheduled as a pediatric tonsillectomy and adenoidectomy and the staff noticed the tonsil tips were clogged/closed directly out of the packaging.A different handpiece was used for the procedure.
