Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: it was reported that the im tibial jig posterior slope adjustment has too much ¿play¿ when attached to the outrigger, impeding the ability to get a predictable and precise setting for tibial posterior slope. No surgical delay was reported. The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection of the returned device found that the i.M rod was missing. However, no damage potentially related to the reported event was observed. The sample only shows signs of a worn appearance consistent with normal and continuous use.Even though a proper assessment was not completely perform since the i.M rod subcomponent was not returned for evaluation. The device was manually inspected and signs of looseness/undesired movement were not observed. The reported condition was not able to be replicated. This condition may occur during the cleaning/sterilization process, particularly when the components are assembled or disassembled. As these procedures are often repeated, there is an increased risk of components going missing. However, without concrete evidence or further information, it is not possible to determine a root cause.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was not confirmed as the observed condition of the hp im tibia jig would have not contributed to the complained issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (a0609) provided is not a valid finished goods lot number.Corrected: h3.
