The following information is alleged: the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2011 due to deep vein thrombosis and pulmonary embolism.Approximately 10 years later the patient had a computed tomography scan which revealed the filter is perforating over 8 mm into the left iliac vein, with multiple other struts perforating the ivc between 9 and 4 mm.Patient also experience pain and distress restricting her ability to engage in activities of daily living.Hospital and medical records have been requested, but not yet provided.Per computed tomography report, "filter type: cook gunther tulip." "filter position: below the renal veins." "filter tilt: yes." "filter penetration: yes." "impressions: the filter is tilted posterior with its proximal cone 4 mm through the ivc wall. The anterior strut penetrates 9 mm through the ivc wall. The left strut penetrates 8 mm through the ivc wall.It penetrates into the left common iliac artery. The posterior strut penetrates 11 mm through the ivc wall. The right strut penetrates 4 mm through the ivc wall.".
