Revision operative report: postoperative diagnosis(es): failed left total hip arthroplasty.Revision of femoral and acetabular components. She was seen and evaluated and being followed for a period of time for accelerated polywear, patient presented with pain.Large uncontained cystic defect at the posterior aspect of the femur and at the medial aspect. This was debrided and appeared to be membrane consistent with osteolysis.Large cystic osteolytic lesions circumferential to the femur.Significant wear and discoloration of the plastic with gross failure at the posterior aspect where the lip was posteriorly.Patient tolerated the procedure well, was transported to recovery room in stable condition. There is no additional information available.

This follow-up report is being submitted to relay corrected information. The following sections were corrected: b1, b2, h6 - health effect - clinical code, component code, type of investigation, investigation findings, h7 and h11 the reported event was unable to be confirmed due to limited information received from the customer. No device was returned for evaluation; further, photographs and/or radiograph images were not provided for review. The patient involved meets the following risk criteria for early prosthesis wear: combination of largest available femoral head and/or thinnest available acetabular liner was used and implanted with a lateralized liner.A definitive root cause was unable to be determined; however, may have resulted from a combination of risk factors such as use error, implant positioning, implant size selection, and patient factors (fitness for surgery, biomechanics, activity level and local tissue oxidation potential). If any further information is obtained that would change or alter any information provided, a supplemental report will be filed accordingly.
