Event or Problem Description
An attempt was made to use one nc euphora rx balloon catheter to treat a non-tortuous, mildly calcified lesion with 90% stenosis in the mid right coronary artery (rca). The device was inspected with no issues.Negative prep was performed with no issues. The lesion was pre-dilated.The device passed through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during delivery. It was reported that balloon deflation difficulties occurred. The balloon was being used to post dilate a stent. It was detailed that many techniques were attempted to deflate the device however the balloon would not deflate at the lesion site. The patient is alive with no injury.

Additional Manufacturer Narrative
Product analysis: the device was returned for analysis. The device returned with a detachment on the hypo-tube.The hypo-tube material appeared to have a clean cut on the detachment site.A kink was evident at the entry port. The distal shaft was necked distal to the entry port and proximal to the shaft.Kinks were evident on the distal shaft.Necking evident to the proximal balloon bond.Balloon returned partially inflated. An attempt was made to inflate and deflate the balloon but due to the necking on the proximal balloon bond, deflation was unsuccessful.Deformation was evident to the distal tip.No other deformation evident to the remainder of the device.Additional information: it was later reported that the balloon was cut on an attempt to deflate.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.

Additional Manufacturer Narrative
Additional information: it was not difficult to remove the protective sheath and the packaging stylette.Balloon deflation difficulties occurred during the first inflation at nominal pressure. The device was not moved or repositioned while inflated. The balloon was being used to post dilate a stent.Techniques included pushing a non-medtronic guide extension catheter over the proximal edge, pulling the balloon and using a wire to try to pop the balloon. The balloon was removed from the patient with difficulty experienced. The patient experienced st elevation during the inflation time. The patient is alive with no further injury.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This statement should be included with any information or report disclosed to the public under the freedom of information act.

Additional information: a patient underwent cardiac catheterization which revealed lesion in the left anterior descending (lac) artery and the right coronary artery (rca).Orbital atherectomy was performed on the rca and a 3.0 x 32mm non-medtronic stent was placed in the proximal rca.It was reported that upon initial inflation the balloon did not dilate. On subsequent inflation there was partial dilation of the balloon, and it was reported that balloon deflation difficulties occurred at nominal pressure. The balloon remained inflated in the rca within the stent, despite going into negative on the insufflator multiple times. After multiple measures the balloon slightly deflated. The patient experienced st elevation on the electrocardiogram (ekg) during the inflation time, but did not complain of chest pain. The st elevations resolved. The patient was asymptomatic post procedure and was discharged the same day.In follow-up one week later the patient reported that their pre-procedure ischemic symptoms had improved.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.
