Additional Manufacturer Narrative
H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.Based on a review of available complaint information and an evaluation of the available sample evidence, the following was concluded: the complaint of a damaged clamp was confirmed; however, the root cause was not identified. The product returned for evaluation was one 4fr dual lumen powerpicc provena catheter.Usage residues were observed throughout the sample.A fracture was observed in the purple-luered lumen clamp.Microscopic inspection of the fracture site revealed a granular fracture surface. No deformation was observed in the vicinity of the fracture.Slight discoloration was observed.Attempts to replicate the observed damage by manipulating the clamp of a non-complainant device resulted in abundant deformation and discoloration. The fracture features were consistent with brittle material failure.Attempts to replicate the fracture features were unsuccessful, suggesting that an additional unidentified environmental factor(s) likely affected the material properties of the clamp.Consequently, this complaint is confirmed as ¿cause unknown¿ at this time.H3 other text : evaluation findings are in section h11.
