A photo was provided of the lens inside the eye.The blue markers for positioning are observed having been placed into the eye for positioning.A dark line can be observed horizontally across the central optic zone. This line may be a crack or a scratch.A final determination cannot be made from this photo.A qualified cartridge was used.A non-qualified handpiece and viscoelastic were indicated.Based on our observation of the attached photos, the lens was inside the eye and a dark horizontal line was observed across the optic central zone. This line may be a cracked area or a scratch. It is difficult to make a final determination without evaluation of the physical sample. The root cause may be related to a failure to follow the ifu (instructions for use).A non-handpiece and viscoelastic were used. The ifu instructs: company foldable iols are qualified for use with an company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination. The ifu instructs that an company qualified delivery system and viscoelastic combination should be used. The use of an unqualified combination may cause damage to the lens and potential complications during the implantation process. The manufacturer internal reference number is: (b)(4).
