Event or Problem Description
According to the reporter, during a laparoscopic inguinal hernia repair, an issue occurred as the mesh was being inserted through the trocar, causing the outer grey seal to completely come off.The surgeon managed to reattach the grey seal using forceps. However, the abdominal pressure could not be maintained. The mesh used became attached to unintended tissue due to the lack of abdominal pressure. There was a rapid loss of abdominal pressure resulted from the device issue. To fully resolve the issue, the surgeon needed to spend additional time removing the mesh from this unintended tissue, leading to a moderate increase in the procedure time, not exceeding 30 minutes.
