This event occurred at (b)(6) hospital in (b)(6), united kingdom.Manufacturer's investigation conclusion: the reported event of a blank flow was confirmed via analysis of the downloaded log file; however, the issue was not reproduced during testing of the returned centrimag flow probe. The log file contained approximately 1 day of relevant data ((b)(6) 2023 per the timestamp).Throughout the log file, the system was observed to be operating the motor at a speed of approximately 2900 revolutions per minute and a flow of approximately 3.2 liters per minute (lpm). On (b)(6) 2023 at 11:53, the flow was observed to gradually decrease from approximately 3.2 lpm to approximately 2.7 lpm.The log file then captured a f3: flow below minimum alarm on (b)(6) 2023 at 11:54 due to the flow dropping below the set flow threshold. On (b)(6) 2023 at 11:55 a f2: flow signal interrupted alarm activated, causing the patient¿s flow value to read as 0 lpm. These alarms were muted and cleared within a minute of activating and correlated to fault flags that may indicate a potential communication issue. The alarms did not affect the system¿s ability to operate the motor at the set speed. The returned centrimag flow probe (serial number: (b)(6)) was evaluated at alongside known working test centrimag equipment, and the returned centrimag console and motor for an extended period of time without any issues or atypical alarms produced, including when the cable was manipulated by hand. The root cause of the reported event could not be conclusively determined through this analysis. The device history records were reviewed, and the records revealed that the centrimag flow probe, serial number (b)(6), was manufactured in accordance with manufacturing and quality assurance specifications. The 2nd generation centrimag system operating manual section 10 ¿ ¿emergencies/troubleshooting¿ provides instructions for operation when there is a need to exchange the main console or motor with a backup console or motor. The recommended practice whenever there is a console or motor malfunction is to replace the console and motor as a set.Remove the blood pump from the malfunctioning motor and console and place the blood pump in the back-up motor and console.Switch all components (console, motor, flow probe and cables) simultaneously to continue patient support, and then perform troubleshooting on the non-functioning system, when it is no longer being used for patient support. The 2nd generation centrimag system operating manual section 12.1 ¿ "appendix i ¿ 2nd generation centrimag primary console alarms and alerts" cover all alarms (auditory and visual), including flow related alarms, and the appropriate actions to take if the issue does not resolve. No further information was provided. The manufacturer is closing the file in this event.
