Event or Problem Description
Medtronic received a report that after resheathing the pipeline once to knock the wings out of the way, the physician was deploying the pipeline 400 x 14.After he pulled the device back to his preferred landing zone upon continuing to deploy he noticed that the pipeline was retracting upon itself. He felt it would be safest to advance the navien over the pipeline and remove the entire system rather than try to continue to deploy. The physician feels that there was some tortuosity at the segment the original pipeline was retracting that may have contributed to the issue. At this point the physician utilized the same triaxial setup and deployed a 2nd pipeline 400 x 16 successfully with no issues. No patient symptoms or further complications were reported as a result of this event.  the patient was undergoing surgery for treatment of a saccular, unruptured aneurysm in the right internal carotid artery with a max diameter of 3mm and a 3mm neck diameter. The landing zone was 3.5mm distally and 3.75 proximally. It was noted the patient's vessel tortuosity was moderate.Dapt (dual antiplatelet treatment) was administered.Pru level was 100.The angiographic result post procedure was the aneurysm was treated successfully.  ancillary devices include a cook shuttle sheath, navien 058 guide catheter, and phenom 27 microcatheter.
