Device report from depuy synthes reports an event in japan as follows: it was reported that during a posterior lumbar interbody fusion (plif) surgery for degenerative disease on (b)(6) 2023, a shaft could not be combined with the t-pal spacer applicator knob.Another t-pal spacer applicator knob was used to complete the surgery. The surgery was completed successfully with no surgical delay. After surgery, it was identified that when the shaft was combined with the t-pal spacer applicator knob while pressing the button which is for removing the shaft, the shaft was combined with the t-pal spacer applicator knob. The t-pal spacer applicator knob in question may be damaged because it should be combined without pressing the button if it was originally turned all the way to the open position. No further information is available. This report is for one (1) t-pal spacer applicator knob. This is report 1 of 1 for complaint (b)(4).
