Event or Problem Description
An endurant ii limb was intended to be implanted during the endovascular treatment of a 55mm abdominal aortic aneurysm. it was reported during the index procedure, during device preparation on the surgical back table, the room circulator opened the outer pouch of the delivery system and handed to the scrub tech. The circulator was wearing a jacket and gloves. The scrub tech proceeded to open the inner pouch. When the device was being prepared a small hair on the inner packaging was observed. It was noted that the hair was small and consistent in size with an arm hair. The device was not used and a replacement endurant limb was used to complete the procedure. per the physician the source of hair cannot be determined but the physician felt the hair was consistent with being left during the packaging process as anyone who came in contact with device was fully covered up.No additional clinical sequelae were reported and the patient is fine.
