The following fields were updated due to additional information: d10: device available for eval yes, d10: returned to manufacturer on: 04-aug-2023.H6: investigation summary two samples were provided to our quality team for investigation. The product was evaluated and a leakage between the vial and protector was observed in one of the samples, no leak was identified in the second sample.Upon inspecting the product it was noted that the puncture within the stoppers of the vial was significantly larger than the spike of the protector.Product undergoes inspections throughout the manufacturing process to ensure the quality and functionality of the device.A device history review was performed for suspected lots 2302125 and 2301131, no deviations or non-conformances were identified during the manufacturing process that could have contributed to this issue.Retained samples of each lot were used for additional evaluation. The protectors were successfully connected to the vials, ensuring they fit properly. The product was evaluated and no damage or defects were observed.Functional testing was performed on the samples, no leak between the protector and vial was identified.Based on our quality team's investigation, we cannot identify a root cause related to our manufacturing process at this time.It is likely the leak was related to the quality of the stopper as well as the assembly of the protector onto the vial. The protector must be attached completely vertical.
