Additional Manufacturer Narrative

H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria. There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: one ultraverse rx pta dilatation catheter in two segments was returned for evaluation. On the visual evaluation, the first segment consists of the portion of the inflation hub and consists of part of the catheter, the second part consists of the remaining detached catheter along with the inner guide wire lumen and balloon. The returned device had multiple kinks along the length of the returned catheter. No functional testing could be performed due to the condition of the balloon. As the balloon was returned in two detached segments for evaluation and no objective evidence for balloon removal difficulty couldn¿t be observed and functional testing couldn¿t be performed due to its condition.Hence the investigation was confirmed for the reported balloon detachment and the investigation remains inconclusive to the reported balloon removal difficulty.A definitive root cause for the reported balloon detachment and balloon removal difficulty could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: (expiry date: 09/2024).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
