Event or Problem Description
An attempt was made to use one 2.00x8mm onyx frontier coronary drug eluting stent (des) to treat a mildly tortuous, mildly calcified lesion with 70% stenosis in the mid obtuse marginal (om). The device was inspected with no issues noted.Negative prep was performed with no issues noted. The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during the delivery. It was reported that the 2.00x8mm onyx frontier appeared to be longer than 8mm.It was detailed that post stent implantation the distal portion of a non-medtronic wire, which was stated to be 15mm was used to measure the stent length. The stent appeared to be approximately three quarters the size of the non-medtronic wire which meant the stent length was estimated to be 12mm.The patient is alive with no injury.
