Event or Problem Description

Per a report from computed tomography (ct), "findings: abdomen vascular: ivc filter is present. The apex of the filter is tilted anteromedially with approximately 8 degrees of tilt medially and 17 degrees of tilt anteriorly. The apex of the filter appears to be near, but not definitively touching the vc wall, although evaluation of the margins of the inferior vena cava are limited without iv contrast. The apex of the ivc filter is located at the level of the bilateral renal veins. No evidence of migration of the filter or stress. One of the medial struts, one of the lateral struts, and one of the posterior struts extends outside of the margins of the inferior vena cava. The lateral strut appears to perforate the edge of the medial wall of the second portion of the duodenum.Millimeters of perforations measured as requested (from the distal tip of the strut to the edge of the inferior vena cava).Lateral strut perforation: 4 mm transverse.10 mm craniocaudad.12 mm in oblique plane along the length of the strut on the coronal image.Medial strut perforation: 6 mm transverse.23 mm craniocaudad.28 mm in the oblique plane along the length of the strut on the sagittal image.Posterior strut perforation: 9 mm ap, 12 mm craniocaudad, and 16 mm in maximal oblique plane on the sagittal image. No visible strut fracture or bending. No visualized ivc stenosis.".
