Event or Problem Description
It was reported that the procedure was to treat a lesion in the left circumflex (lcx) artery with 90% stenosis, no calcification and mild tortuosity. The versaturn guide wire was used in the lcx and noted to have stretched and got caught on the stent's strut during removal.Another versaturn guide wire was used to treat the obtuse marginal (om) and had stretched coils probably from the first guide wire. The guide wires were removed independently. It was noted on both versaturn guide wires that the core and coils were not intact. The versaturn guide wire used in the lcx was noted to possibly have fragments of coil on the versaturn guide wire used in the om.There was no adverse patient effect and there was no clinically significant delay in the procedure. No additional information was provided.

Additional Manufacturer Narrative
A visual inspection was performed on the returned device. The reported stretched coils were confirmed. The reported break could not be confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history of the reported lot did not indicate a lot specific quality issue. The investigation determined the reported difficulties appear to be related to the operational context of the procedure. In this case, the procedure was using two guide wires, one in the left circumflex artery (lcx) and the reported wire was used in the obtuse marginal (om).It was reported that the guide wire used in the lcx likely interacted with the reported guide wire, resulting in the reported stretched/damaged coils.Additionally, return analysis noted that there was no coil break or separation as reported. There is no indication of a product quality issue with respect to manufacture, design, or labeling.
