(b)(4) date sent: 9/28/2023 investigation summary the product was returned for evaluation.Visual inspection was conducted on the returned device.Visual analysis of the returned sample determined that the 0037h device was returned with the ptfe insulation detached and returned. In addition, the coating of the tip electrode was damaged. The electrode was tested for continuity and passed the test.As part of ethicon endo surgery¿s quality process, all devices are manufactured, inspected, and released to approved specifications. The most likely cause of the coating damage is caused by operating the electrosurgery equipment (generator) at high power settings with continual activation of the electrode for long periods or by placing the electrode tip against another instrument causing extreme heat which results in the melting of the tip.No definitive root cause could be reached that might have caused the detachment of the ptfe.Manufacturing record evaluation is not required as the reported event is not associated with the manufacturing process and/or the potential cause of the defect cannot be associated to manufacturing.
