H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.Based on a review of this information, the following was concluded: the complaint of a leak is confirmed; however, the root cause could not be determined. One photograph and one video of a powerglide pro catheter was returned for evaluation. An initial visual observation of the photograph showed a syringe attached to the catheter.Evidence of a leak could be seen on the cloth beneath the catheter. An initial visual observation of the video showed the catheter being flushed. The device was leaking at the junction of the pink molded joint and the catheter tubing.Use residue was observed on the sample in the returned photograph and video. No other details of the damage were observed on the sample in the returned photograph and video. This complaint will be recorded for future trending and monitoring purposes.H3 other text: evaluation findings are in section h11.
