Additional Manufacturer Narrative
22-sep-2022 two typos occurred in the description of the initial mdr regarding the first two dates mentioned. The first three sentences should read: on (b)(6)2022 a pk papyrus covered stent system was selected for treatment of a type iii lesion in the mid rca.It was implanted without problems, and the perforation was successfully sealed. On (b)(6)2022 patient presented to er after an out of hospital cardiac arrest. He was treated with medication and was monitored but was in no condition to go to the cath lab, and his overall prognosis was very poor. The affected pk papyrus stent system was not returned to biotronik and could therefore not be subjected to a technical investigation.Images of the case such as angiographies could also not be obtained. The provided clinical information (pk papyrus device registration forms of both the initial procedure and the second hospital visit, interventional cardiology notes, cathlab report, ekg, medical death note and email correspondence) was reviewed to establish whether a device deficiency contributed to this event.Review of the product release documentation confirmed that the device was manufactured according to specifications and fulfilled all the requirements of in-process and final inspection. No device deficiency and no manufacturing related root cause was identified.Pk papyrus is indicated for the treatment of acute perforations of native coronary arteries and coronary bypass grafts in vessels 2.5 to 5.0 mm in diameter. As such, this device is typically used in emergent response to a life-threatening adverse procedural event unrelated to use of the pk papyrus.
