H3, h6: the associated device was returned and evaluated.A visual inspection of the returned device reveals multiple scratches, gouges and discoloration on the device. These issues could be from the insertion attempt or removal attempt.A review of complaint history revealed similar events for the listed device over the previous 12 months, but no similar events for the batch based on the historical data, this failure mode will be monitored for future complaints for any necessary corrective actions. The clinical/medical evaluation concluded that as of the date of this medical investigation, the requested supporting clinical documentation has not been provided; therefore, there were no clinical factors found which would have contributed to the reported loosening and subsequent revision. The provided photos were reviewed, but do not aid in determining a clinical root cause for the reported pain and joint effusion. It is noted, ¿it was noticed that there was a complete de-bonding of the gns ii cmt tib size 6 right from the cement mantle.¿ the provided implant photo appears void of cement. With the limited information provided, the patient impact beyond the reported revision cannot be determined. No further clinical assessment is warranted at this time.A review of the manufacturing records did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident. At this time, we do not have reason to suspect that the product failed to meet any product specifications at the time of manufacture.A review of the risk management and information for use files revealed this failure mode was previously identified. The anticipated risk level is still adequate.Assessment of historical escalated cases concluded that there are no prior actions related to this device and failure mode.A review of instructions for use for knee systems revealed in possible adverse effects that looseness of components has been identified as a result from trauma, improper implant selection, improper implant positioning, improper fixation, and/or migration of the components.Muscle and fibrous tissue laxity can also contribute to these conditions. Also, revealed that temporary or permanent nerve damage can result in pain or numbness of the affected limb.Factors that could contribute to the reported event include abnormal motion over time, bone degeneration, size selected, inadequate integration between the cement and bone/implant, osteolysis and/or traumatic injury.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor future complaints and investigate as necessary. We consider this investigation closed.
